BARC-CT Spay/Neuter Contract

THE FOLLOWING SPAY/NEUTER CONTRACT PERTAINS TO THE ADOPTION OF UNALTERED PUPPIES

Please read the following text carefully, you will be required to agree to these terms if you are adopting an unaltered animal.

The adopted dog, as described above, will be altered by BARC CT’s veterinarian on an agreed upon date for spay/neuter surgery. I am responsible for the transporting of my adopted dog to and from the vet clinic. If I choose to use my own veterinarian, I am responsible for all costs associated with the spay/neuter of my pet and that BARC CT bears no obligation for these costs. I understand that I must provide proof of spay/neuter from a licensed vet by six months of age. I understand that by voluntarily signing this agreement, I am entering into a legal and binding contract with BARC CT—Bridgeport Animal Rescue Crew of Connecticut. Breach of any term(s) of this agreement is deemed actionable by BARC CT. In the event there is a violation of the agreement, I agree that I must return the dog without reimbursement of any kind. I certify that I have read and understand the terms of this Spay/Neuter Agreement. I acknowledge that failure to comply with these terms shall be considered a breach of contract and shall result in the immediate return of the adopted dog to BARC CT and may result in civil penalties for which I will be responsible.
